 What is Hospice?

  • Plumas Community Hospice is a volunteer organization that supports patients who are facing the end of life and also helps their families as they cope with their pending loss.

 SERVICES

  • All- volunteer Board Of Directors includes a physician, registered nurse, trained volunteers and spiritual leaders.

  • Hospice works with Home Health toward the goal of having the patient remain at home whenever possible with the emphasis on pain management and comfort measures.

  • Caregiver education, bereavement workshops and support groups.

  • Respite care: support and practical assistance from trained volunteers.

  • Some medical supplies & equipment.

 Who is Eligible For Hospice?

  • Any patient who has been diagnosed with a terminal illness.

  • A patient whose primary physician approves and works with the Hospice team.

  • A patient and family who choose comfort (palliative care) rather than aggressive curative treatment

 

  • We serve patients both in home & hospital settings

  • To honor patient directed care

 There is no fee for our Hospice Services   

We are NOT a Medi-care certified hospice
